Position Summary

About the role:

An exciting opportunity exists to join our Home Care Packages team, providing a high quality of care to our clients. As a Client Advisor you will work collaboratively with clients to develop flexible, personalised and culturally appropriate goal directed plans. You will also be responsible for conducting annual reviews with clients, including revising their care plan, goals, services and budgets.

This is the place for you if you:

  • Hold a current driver’s licence.
  • Hold a tertiary qualification in Allied Health/ Community Services, or related discipline
  • Are known for your caring and compassionate nature
  • Are reliable, adaptable, motivated and committed to providing quality care 
  • Have experience in the care advisor role (or care management equivalent),  preferably within the Home Care Packages Program
  • Thrive in a team and also appreciate the one to one interactions
  • Are committed to being part of a workplace culture based on the BEST way of life (Believe, Excel, Support and Trust) 
  • Are willing to work within the ethos of the Uniting Church

You will need:

  • A current Working With Children’s Check 
  • Full Australian working rights 
  • A current Police Records Check or be willing to undertake this process 
  • A current flu vaccination and be prepared to meet this annual requirement
