Position Summary

Team Leader Home Care Healthy Communities - Social Support 

  • Full time Team Leader position based in Hawthorn and also leading programs in Kingsville and Geelong

About the role:

A rewarding opportunity exists for a dynamic individual to join our Home Care team, and work in conjunction with the Program Manager to lead the development and operational management of innovative and client directed social support services to older people and their families living in the communities of Hawthorn, Kingsville and Geelong.

This is the place for you if you have:

  • Relevant qualifications in Community Care, Leadership or a related discipline; and or experience in management in Health or Aged Care
  • Strong leadership skills with the ability to lead a successful team
  • Experience in establishing and maintaining professional relationships with clients, their families and representatives, and other community providers 
  • Comprehensive knowledge of Best Practice and experience in assessment, care planning and service delivery for older people living in the community
  • Thrive in a team and also appreciate the one to one interactions
  • Commitment to being part of a workplace culture based on the BEST way of life (Believe, Excel, Support and Trust)
  • Are willing to work within the ethos of the Uniting Church

You will need:

  • A current Working With Children’s Check 
  • Full Australian working rights
  • A current Police Records Check or be willing to undertake this process
  • A current flu vaccination and be prepared to meet this annual requirement
  • A current Victorian Drivers Licence 
  • To be willing to use your own registered and comprehensively insured vehicle, if required.
